Enrollment Checklist
✓ Child’s Birth Certificate (1 yrs and up)
✓ Hospital footprints (6wks-1yrs)
✓ Parent State ID or DL
✓ Parent’s Social Security Card
✓ Child’s Current Physical with Shots, TB Test, and Lead Test Results
✓ Vision screening (3yrs and up)
✓ Child’s Dental Exam (2yrs and up)
✓ IL Action for Children Current Approval & Change of provider
✓ Supply Fee-$100 per child
